Practice Accepting Medicare Patients

Hanover — Dartmouth Health Connect, the practice that provides primary care to employees and their adult dependents of Dartmouth College, the New England Carpenters Health Benefits Fund, and some employees and their dependents of King Arthur Flour, is now accepting Medicare patients.

Patients can take advantage of the practice’s longer appointments and 24-hour communication access.

Dr. Joel Lazar, the medical director and a primary care doctor at the practice, described the addition of Medicare patients served by the Hanover practice as a “really important stride in moving primary care forward to meet the needs of the population.”

Medicare is government-funded health care for people age 65 and older.

Dartmouth Health Connect, which opened in 2012 in Hanover, is an Iora Health practice. The Boston-based health care system operates practices across the country.

Current Dartmouth Health Connect patients who are approaching their Medicare eligibility years can insure that the “relationship and continuity” the practice has developed can continue, which “older individuals especially need,” Lazar said. It is also available to new patients.

When a new patient joins the practice, staff speak to them about their health priorities.

Each patient is also assigned a health coach who can “help patients to articulate their goals and partner with individuals in achieving goals.”

Staff at the practice also help patients navigate receiving care from specialists.

The practice also offers 24-hour access through phone “so that we can problem solve in multiple different ways,” Lazar said.
