April 11-- Apr. 11--Food Establishments With Violations: Red/Blue/Total

Burger King, Centralia 30/0/30

Several food workers did not have valid food worker cards available. Please correct within one week and develop procedures to ensure all cards are kept current. (5 red)

Many pans of lettuce and sliced tomatoes were incorrectly managed under a Time as a Control plan, which could have allowed these potentially hazardous foods to be held at unsafe temperatures for longer than the maximum time limit. Pans were marked for times greater than four hours and without ensuring they were 41 F or below. Times for tomatoes must be either start when the first tomato is sliced or when they reach 41 F or below. These were either discarded or relabeled. This is a repeat violation. (25 red)

Inspection: April 2

Jeremy's, Chehalis 35/5/40

Gravy and au jus were hot holding between 111 and 120 F and must be kept at 135 F at all times. Based on time since reheating, these were reheated. (25 red)

Several potentially hazardous foods were cold holding between 42 and 51.7 F and must be kept at 41 F or below at all times. These were either discarded or transferred to cool to 41 F. This is a repeat violation. (10 red)

The main dishwasher was not sanitizing (chlorine bucket too low) and there were no test strips to test the hot water sanitizing dishwasher in the bar. Please provide 160 F test strips, test at least weekly and keep a date log with the strips available for inspection. (5 blue)

Due to the third cold-holding violation, an operational plan is required and must detail how all violations noted in the previous two years have been or will reliably be corrected. This plan and associated $250 fee must be submitted within 10 days.

Safeway grocery and produce, Centralia 20/0/20

Produce was still not being washed and sanitized according to procedures, with berries being immersed in sanitizer longer than allowed. These were discarded. This is a repeat violation. (10 red)

Required operational plans were not submitted following the last inspection. Please submit within one week. (10 red)

Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Blue violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection.

Any establishment receiving 40 red points or any red point item repeated within an 18 month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points (red and blue) on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.
