-Rice was held at 127°F, in a hot holding unit, rather than 135°F or above as required. Reheat the rice to 165F immediately. Monitor food during hot holding to ensure that the food stays at or above 135F. *CORRECTED*

-Food was stored open with no covering in the walk-in cooler, walk-in freezer and in an upright freezer. Cover all food immediately and at all times during storage. Raw chicken and shell eggs were stored above sprouts and vegetables in the walk-in cooler. Store raw meats and eggs on the lowest shelves below vegetables and ready-to-eat foods. Correct immediately.
